2. The death certificate of late Dr.
Logan Adura (Her deceased father) confirming his death.
3. A copy of the last statement of
account issued to Dr Logan Adura by our bank.
4. Your valid contact address and name.
5. Your personal phone and fax numbers.
6. Scanned copy of your national
identity or international passport.
Note that the above are compulsory, and
are needed to protect our interest, yours, the next of kin during and
after the claims. These shall also ensure that a smooth, quick and
successful transfer of the fund is made within 48 hours from when we
receive the above mentioned documents.
